Write it up before the score
After the session you write the SOAP note, record any measure you gave, and document risk. No debrief or score until it is written up.

Your formulation, your plan and your note are each checked against the case, and every section shows its result before you open it. The score comes with how much you spoke and what you asked.
Follow one patient across sessions
Episodes follow a patient session by session, with outcome measures like PHQ-9 and a formulation you revise as you go. Only measures you administered appear. Nothing is interpolated.
